Dr. Lee House Calls accepts:
Dr. Lee House Calls is not a health insurance provider and does not bill insurance. However, he can provide a super bill (itemized receipt with insurance codes) so you can personally submit to insurance for reimbursement. He does not guarantee nor does he assist in the process of attempting reimbursement.
A booking fee of $50 for new patient appointments must be paid electronically in order to book your appointment. The $50 will be applied towards any service rendered during that patient visit. Remaining balances must be paid at the time of services rendered. If the visit is cancelled less than 24 hours before the appointment, the booking fee will be forfeited. If the original booking fee is forfeited, the booking fee must be paid again prior to rescheduling your new patient appointment.
Because of the travel and extra time requirements of the house call model, efficient scheduling is a top priority. A fee of 50% of the amount of scheduled appointment will be charged for missed appointments and appointments that are cancelled or rescheduled with less than 24 hours notice.
This fee will start at $10 and increases the further your location & time it takes to travel to. This fee will be charged only once per house call. For example, if 2 people are treated during a house call, the $10 convenience & travel fee is charged only once so each person would split the $10 fee ($5/person). The more people are treated during a house call, the lower the fee will be per person.
Dr. Lee can apply Kinesiotape / Rocktape for an extra fee of $15 per region.
