Payment Options

Dr. Lee House Calls accepts:

Insurance

Dr. Lee House Calls is not a health insurance provider and does not bill insurance. However, he can provide a super bill (itemized receipt with insurance codes) so you can personally submit to insurance for reimbursement. He does not guarantee nor does he assist in the process of attempting reimbursement.

New Patient Booking Fee

A booking fee of $50 for new patient appointments must be paid electronically in order to book your appointment. The $50 will be applied towards any service rendered during that patient visit. Remaining balances must be paid at the time of services rendered. If the visit is cancelled or rescheduled more than 24 hours before the appointment, you will receive 100% refund. If it is less than 24 hours before the appointment, the booking fee will be forfeited. If the original booking fee is forfeited, the booking fee must be paid again prior to rescheduling your new patient appointment.

Cancellation Policy

Because of the travel and extra time requirements of the house call model, efficient scheduling is a top priority. A fee of 50% of the amount of scheduled appointment will be charged for missed appointments and appointments that are cancelled or rescheduled with less than 24 hours notice.

Travel Fee

This fee is for the convenience of having the doctor travel to you. It is time consuming to travel to a conventional office, sit in a waiting room all for just a few minutes with the doctor. This fee eliminates that hassle by having Dr. Lee travel to you with his portable chiropractic table & equipment to provide convenient, house call chiropractic. Dr. Lee is based in Metairie. The travel fee increases depending on the time of day, traffic, & time it takes to travel to your location. See below for the estimated fees.

Metairie & Kenner: $30

Harahan & River Ridge: $30

New Orleans: $30-$75

Westbank, Chalmette, Laplace, Destrehan, & St. Rose: $75

This fee will be charged only once per house call. For example, if 2 people are treated during a house call and the convenience & travel fee is $50, it would be $25/person; if 4 people are treated then it would be $12.50/person. The more people seen during a house call, the lower the fee will be per person.

New Patient Visit 30 min. - $100 (generally for patients with no to minimal symptoms, Maintenance/Wellness care)

30 minute visit includes set up and breakdown of equipment, examination, and treatment. **If Dr. Lee’s exam indicates x-rays or MRI imaging is needed prior to the adjustment, he will refer out for imaging and the adjustment will be performed on the 2nd visit depending on imaging findings.

Health history (intake forms) must be filled out 24 hours prior to appointment. Any late submission of intake forms may result in rescheduling of appointment, decreased treatment time, or increased cost due to required time to review forms.

New Patient Visit 45 min. - $150

45 minutes visit includes set up and breakdown of equipment, examination, and treatment. **If Dr. Lee’s exam indicates x-rays or MRI imaging is needed prior to the adjustment, he will refer out for imaging and the adjustment will be performed on the 2nd visit depending on imaging findings.

New Patient Visit 60 min. - $200 (additional fee for over 60 min.)

1 hour visit includes set up and breakdown of equipment, examination, and treatment. **If Dr. Lee’s exam indicates x-rays or MRI imaging is needed prior to the adjustment, he will refer out for imaging and the adjustment will be performed on the 2nd visit depending on imaging findings.

Follow Up Visits

Follow up visits are range from $80-$200 + Travel Fee

Re-Examinations - $100-$200 (time based)

**Re-examinations will be conducted for actively treating patients who suffer a new injury or inactive patients who suffer a new injury, exasperation of original symptoms, or have not received treatment for over 2 months. Similar to a new patient evaluation, proper documentation and paperwork are necessary, examination and strategic plan will be conducted and treatment will begin if no imaging is required (X-rays or MRI).

**This visit will be based on the total time it takes to complete the re-examination and treatment.

Kinesiotape / Rocktape

Dr. Lee can apply Kinesiotape / Rocktape for an extra fee of $15 per region (Patient will provide supplies. Dr. Lee can provide supplies for extra fee)

1 hour visit includes examination and treatment. **If Dr. Lee’s exam indicates x-rays or MRI imaging is needed prior to the adjustment, he will refer out for imaging and the adjustment will be performed on the 2nd visit depending on imaging findings.
